Board disciplines local nurses

Two area nurses were recently disciplined by the state Board of Nursing.

Licensed practical nurse Michelle Simmons of Linden was placed on probation for at least two years, according to online court records.

According to a letter from the board dated Nov. 6 to Simmons, the nurse may have committed numerous violations.

These include while working at Dutch Haven Assisting Living in Maurertown about a year ago, Simmons didn't properly check a patient's morphine order, which ended up in his getting a wrong dose; on an employment application at Golden Living Center in Berryville in May, not stating she'd been fired from Dutch Haven; and while working at the Berryville facility, diverting medications meant for patients.

The letter states that in August, Simmons had documented she'd given various drugs to patients, but the patients denied getting them. The drugs involved include Dilaudid, Lortab, Norco, Xanax and Percocet. Board records indicate Simmons was fired from Golden Living in August.

According to a Dec. 28 order from the Board of Nursing, Simmons received her license in 2011, but had since been fired from three nursing jobs. She has told the board she takes care of three patients now through a home healthcare program, and teaches phlebotomy at a community college, the order states.

The order sets up several conditions for Simmons to follow, including telling all current and future employers about her probationary status and giving them a copy of the order, obtaining quarterly evaluation reports from her employers, only working in a facility with on-site supervision from either a doctor or a nurse and being evaluated by a mental health/drug dependency specialist.

Earlier in December -- Dec. 14 -- another nurse, Denise Bain, a registered nurse from Cross Junction, was placed on indefinite suspension, according to the Board of Nursing's online records.

According to an order from the Board of Nursing, Bain showed up April 20 to her job at Fauquier Hospital in Warrenton more than four times the legal driving limit. Her blood alcohol content was measured at nearly .37, and she was fired April 25, the order states.

Bain had been reprimanded by the board April 17, according to online records, after showing up to work at Winchester Medical Center drunk in August 2011. Her blood alcohol content was .236, according to the earlier order.

Winchester Medical Center fired Bain, who had worked there for about eight years, according to the April order. It states Bain explained she had drunk several shots of whiskey earlier in the morning in response to severe foot pain, and didn't want to leave her unit short-staffed.

Neither woman has a public phone listing.
